Overview

As a CNA in this role, you’ll bring your certification into the home setting — taking vital signs, observing changes in client condition, assisting with mobility, and documenting care notes. You’ll often work under the direction of a care manager or supervising nurse but operate independently in the home, where the rhythm is slower than facility work but the relationship with each client runs deeper.

Responsibilities

• Drive seniors to appointments, meetings, and church services
• Assist with bathing and grooming
• Provide incontinence care management
• Perform light housekeeping to reduce stress in the home
• Prepare meals to keep seniors healthy
• Administer medication reminders to avoid hospital readmissions
• Ensure safety with memory care strategies
• Assist with physical therapy exercises

Qualifications
